POSITION PURPOSE:

The Primary Care Behavioral Health Consultant (BHC) will serve as a core member of the interdisciplinary Patient-Centered Primary Care Team, in the Primary Care Behavioral Health Program at Aviva Health to improve patients health outcomes.If you are working towards your licensure, we provide a positive, supportive experience for you. SHIFT and SCHEDULE:

• 4 x10- hour shifts or
• 5 x 8-hour shifts
• No On-Call
• Day Shift
• Monday to Friday

WORK SETTING:

• Primary Care
• Clinic

ESSENTIAL FUNCTIONS:

• Serves on the multidisciplinary primary care team providing primary care behavioral health services.
• Conducts warm hand-off assessments.
• Utilizes excellent clinical judgment in assessing patients mental/ behavioral health condition(s) or immediate needs.
• Applies high-quality, appropriate brief, short-term treatment interventions based on clinical diagnosis and assessment of the patients presenting problem(s).

QUALIFICATIONS:

• Minimum of masters degree in Clinical Social Work (MSW) or Masters degree in Professional Counseling or Marriage and Family Therapy. Or, Ph.D. or Psy.D. in clinical or counseling psychology.
• Current and active licensure in the State of Oregon or license eligible within 6 months of hire.
• Or, CSWA or Registered Associate in Professional Counseling or Marriage and Family Therapy, working towards Oregon licensure as an LCSW, LPC, or LMFT; or Psychologist Associate; or QMHP certification.
• Passion for serving the underserved.
